What does the PLU Student Health Insurance Plan cover?

You can review coverage details of the PLU Student Health Insurance Plan online  For any questions regarding coverage, please call  EIIA, our plan administrator, at 800-537-8410 extension 214 or email your questions to an EIIA plan adminstrator.


How will I be billed for health insurance?

Full-time undergraduate students and international students will see the health insurance fee in their fall semester billing. Or spring billing, if starting study in spring.  After a student waives-out online, the charge will be reversed and show in future bill or statement.


I have health insurance and do not want supplemental coverage. How do I waive out of the plan?

Each year students will be automatically enrolled in the PLU Student Health Insurance Plan. To decline the plan, each year students will need to provide evidence of comparable primary insurance through an online waive out form.

Students starting study fall semester 2008 must waive out between April 1, 2008 and no later than September, 19, 2008, midnight. No waivers will be accepted after September 19, 2008.

Waive Out Instructions:
1. Have your current health insurance policy information on-hand.
2. Access the online waiver form and click "Waive Student Insurance" in the left column.
3. Answer the student medical insurance questionnaire.
4. You will receive a confirmation email once you have completed the online waive out process. Print-out the email confirmation for your records. Should questions arise, this email confirmation is the only documentation PLU accepts to verify completion of the waive out process.
5. After waiving out, a future bill or statement will show the insurance plan fee reversed.
6. PLU reserves the right to determine if plans are comparable.


When is the deadline for submitting a waiver?

Do I have to fill out a health insurance waiver each year?

Yes, you must complete a health insurance waiver each year as part of the registration and billing process. Health insurance plans often change coverage annually.


Will I receive a health insurance identification card?

Yes, students enrolled in the PLU Student Health Insurance Plan can download, and print, a health insurance identification card from the EIIA Student Program Website.  Intercollegiate health insurance ID cards are available through the PLU athletic trainer.

What if my insurance circumstances change during the year?

If you experience an involuntary loss of other health insurance coverage during the year, you can enroll in the Student Health Insurance Plan at PLU Student Services. To enroll, you will need to provide them a letter from your previous carrier stating the involuntary loss of your coverage.


Can I use this plan as secondary  insurance?
Students who have existing health coverage under other insurance programs may consider participating in the PLU Student Health Insurance Plan as a supplemental coverage to their primary insurance plan.


Who do I contact regarding benefit details?
EIIA is our insurance plan administrator. They are the experts on the insurance plan and coordinate the benefits with the insurance company. All questions with regard to coverage may be addressed to EIIA at 800-537-8410, extension 214 or via email.


Who do I contact regarding referrals or claim processing?
The PLU Health Center should be your first visit for health care as  the primary care provider. They will provide referrals to other care providers when needed. A referral from the PLU Health Center is required in order to receive reimbursements for covered medical expenses. 

When a student is away from campus, during summer for example, the insured student can submit a claim form directly to the insurance company for reimbursement of covered medical expenses obtained through his or her local medical clinic, emergency room, or physician according to benefits schedule.

Claims instructions and forms for medical expense reimbursements are available online. The PLU Health Center will provide you a signed claim form after visits and for referrals.
