The address confirmation form is required for all PLU students. It is the student's responsibility to make any necessary updates regarding the permanent, billing, mailing and parent addresses.
If PLU is unable to locate a student at any addresses supplied by the student, and PLU has not been notified of any address changes, the student's account will be placed on an address hold.
An address hold will prohibit further registration and any other PLU services until the necessary information has been updated.
Please return the completed address confirmation form to the Student Services Office in Hauge Administration Building, Room 104.
If you live in a campus residence, your campus address will be maintained by Residential Life.
The form is also available on the wall display across from the Student Services Center.
