Pacific Lutheran University (PLU) requires all international students to maintain medical insurance that provides coverage in the United States and meets certain minimum benefit requirements. To ensure this, PLU will automatically enroll all international students in PLU’s International Student Accident and Sickness Plan (Student Insurance Plan). This insurance premium will automatically be added to your university bill at time of registration. Students who have insurance provided by their government are eligible to waive this insurance. You must provide proof that benefits provided by your government policy are at least equal to those required by Pacific Lutheran University. This compliance form must be used to provide this information to the university.

International Student Insurance Plan Waiver 2016-2017

If the waiver is received after the deadline or does not provide proof of coverage of the benefits listed, the waiver will be denied and the student will be responsible for paying the enrollment fee for PLU international student accident and sickness plan.

The international student insurance plan waiver must be submitted to the Office of Finance and Administration no later than September 6, 2016. Waivers will not be accepted after the date.

Cost and Billing

PLU will register students in the PLU International Student Injury and Sickness Insurance Plan made available through International Health Consortium. The $1,103 plan fee will be charged to the student account. Highlights of the coverage will be posted on the Finance and Administration website.

Care and Coverage

The primary care provider will be the PLU Health Center. A referral is required for payment of outpatient benefits. When the Health Center is not open or the student is away from campus, during summer for example, the insured student may submit a claim form directly to the insurance company for reimbursement of covered medical expenses obtained through his or her local medical clinic, emergency room, or physician according to benefits schedule.

The insurance coverage period for students enrolling for the full academic year is August 15, 2016 to August 14, 2017. International students who plan to travel in the U.S. prior to the coverage start date or after the end date should purchase travel insurance to cover any incidents that may occur during those time periods.

An accident or injury must be treated within 90-days. Claims will be accepted up to 180-days following accident, illness, or injury. Instructions for claiming reimbursement of your medical expenses is online at the EIIA Student Program Website.
