Medical health insurance is required for all full-time and part-time international students. Each year students will be enrolled automatically in the PLU International Student Health Insurance Plan. A student who is covered by a comparable health insurance plan can waive out of the PLU Student Health Insurance Plan by providing evidence of comparable primary insurance.
International students must have their insurance company submit the International Student Health Insurance Plan Waiver no later than September 17, 2012. No waivers will be accepted after midnight on September 17, 2012.
The plan provides important coverage for office visits, prescriptions, emergency treatment and hospitalization.
International Students: The 2012-2013 fee for the PLU International Student Health Insurance Plan is $825 for the academic year, $435 for students enrolling for one semester only. This insurance plan meets best practices recommended by the US State Department.
Billing: The insurance plan fee will be billed as part of the fall billing process, or spring billing for those who start spring semester. After waiving-out, a future bill or statement will show the insurance cost reversed. Financial aid resources can help cover the cost of the insurance plan.
Health Insurance ID Cards: Health insurance identification cards will be available through the PLU Student Health Center.

Waive Out Instructions for International Students:
1. Complete the top portion of the International Student Insurance Plan Waiver.
2. Ask your insurance company representative to complete the remaining portion of the International Student Insurance Plan Waiver.
3. Your insurance company will submit the completed form to the Office of Finance & Operations via mail: Pacific Lutheran University, Attn: Finance & Operations, Tacoma, WA 98447 or via fax: (253) 536-5047.
4. After the waiver is approved, the next bill or statement will show the insurance plan fee reversed.
5. If the waiver is denied, the student will be notified by the Office of Finance & Operations.
An accident or injury must be treated within 90-days. Claims will be accepted up to 180-days following accident, illness, or injury. Instructions for claiming reimbursement of your medical expenses is online at the EIIA Student Program Website.
