Exactly what is it that I will get for my $150 per semester?

Here are some examples of the financial and health care benefits that you will receive under the Wellness Access Plan:

I have an illness or accident that requires an emergency room visit or CT scan.

  • I have health insurance: Your insurance plan would provide first dollar coverage.  The PLU Wellness Access Plan would provide up to $1,000 in benefits to reimburse unmet deductible or eligible out-of-pocket expenses or up to $5,000 per accident.*
  • I do not have health insurance: The PLU Wellness Access Plan would help you pay for the first $1,000 of the cost of the emergency visit or test or up to $5,000 per accident.*

I need a physical to be cleared for study away, a job, a sports team, or co-curricular program.

  • I have health insurance: There is no charge under the Wellness Access Plan for a physical exam at the PLU Health Center and completion of the accompanying paperwork.
  • I do not have health insurance: There is no charge under the Wellness Access Plan for a physical at the PLU Health Center exam and completion of the accompanying paperwork.

I have a chronic illness for which I need monthly medical visits.

  • I have health insurance: The Wellness Access Plan would cover up $1,000 total for the year to supplement any co-pay obligations under your existing insurance plan or other eligible out of pocket costs.*
  • I do not have health insurance: The Wellness Access Plan would cover up $1,000 total for the year to assist with the costs of your care.*

I need to see a provider off-campus.

  • I have health insurance: If you need to see a provider that is off-campus, whether in or out of your primary plan network for outpatient care, the PLU Wellness Access Plan can reimburse you for the cost of the medical expense up to $1,000 per sickness and $5,000 per accident.*
  • I do not have health insurance: If the PLU Health Center needs to refer you to a provider that is off-campus, the PLU Wellness Access Plan can reimburse you for the cost of the medical expense up to $1,000 per sickness and $5,000 per accident.*

I was sick in the fall with one illness and used the PLU wellness access plan, but in the spring, I got sick with a different illness and need to see a different specialist off campus.

  • I have health insurance: The benefits provided by the PLU Wellness Access Plan are per illness (diagnosis).  Therefore, each illness (diagnosis) is subject to a new limit.  So The PLU Wellness Access Plan would provide up to $1,000 in benefits to reimburse unmet deductible or eligible out-of-pocket expenses.*
  • I do not have health insurance: The benefits provided by the PLU Wellness Access Plan are per illness (diagnosis).  Therefore, each illness (diagnosis) is subject to a new limit.  So The PLU Wellness Access Plan would provide up to $1,000 in benefits to help pay for the costs incurred for the spring illness.*

* “Costs” and “out of pocket costs” refer to those fees charged by a healthcare provider or facility.

I need to see a specialist off campus, but I have not met my deductible yet.

  • I have health insurance: The PLU Wellness Access Plan will supplement your primary plan by reimbursing you for the unmet deductible up to the maximum benefit under the PLU Wellness Access Plan.
  • I do not have health insurance: The PLU Wellness Access Plan will provide your reimbursement for your out of pocket costs up to the maximum benefit under the PLU Wellness Access Plan.
