Fair Registration Form (required)

Please click on the the link below, fill in all applicable places and submit. Teams: Follow the instructions on the form for registering team members.

• Online Registration Form. CAUTION: When using the online registration form, the first email line must use a different email address for each registrant, even if the participants are registering in different grade levels. If the same email address is used to register several participants, even if they are registering in different grade levels, the latest registration replaces each previous one.

ISEF Forms (Required): All ISEF Forms (including an Abstract) necessary for your project must be submitted by Wednesday, March 13, 2013. See ISEF Rules Wizard for a help in determining what forms are required for your experiment. You may save  the forms, type in the information, print, obtain the proper signatures, and mail to:

All projects are required to have following forms: 

• Form 1: Checklist for Adult Sponsor/Safety Assessment Form

• Form 1A: Student Checklist Form/Research Plan

• Form 1B: Approval Form

• Abstract Form

Some projects may require additional forms:

• Is your project a continuation in the same field of study as a previous project? If your answer is yes, you need:

  • Form 7: Continuation Form

• Are you doing research at a university lab, medical center, research lab, or anything along those lines? If your answer is yes, you need:  

  • Form 1C: Regulated Research Institutional/Industrial Setting Form  

  • Form 2: Qualified Scientist Form

• Does your project involve humans as subjects? If your answer is yes, please read the Human Subjects Risk Assessment Guide for information. You will need: 

  • approval by a committee (institutional review board or IRB) consisting of a medical health professional, school administrator, and an educator prior to experimentation and complete Form 4: Human Subjects and Informed Consent Form

  • an informed consent document to give to your human subjects (if the human subject is under 18, their parent or guardian must sign this form too). See the Informed Consent Form

  • Form 2: Qualified Scientist Form (if applicable)

• Does your project involve vertebrate animals? If your answer is yes you must contact the fair director NOW and describe your experiment. You will need: 

  • Form 5: Vertebrate Animal Form (5A or 5B) prior to experimentation 

  • Form 2: Qualified Scientist Form (if applicable)

• Are you working with potentially hazardous biological agents? If your answer is yes, you need: 

  • Form 3: Risk Assessment Form

  • Form 6A: Potentially Hazardous Biological Agents Form (for all studies involving microorganisms, rDNA, and fresh tissues, blood and body tissues)  

  • Form 6B: Human and Vertebrate Animal Tissue Form (for all studies involving fresh tissues, primary cell cultures, blood, blood products, and body fluids)

  • Form 2: Qualified Scientist Form (if applicable)

• Are you working with hazardous chemicals, activities, or devices? If your answer is yes, you need: 

  • Form 3: Risk Assessment Form

  • Form 2: Qualified Science Form (if applicable)
