1 > Fill out and submit the player
information sheet here
2 > Apply for admission to PLU
- Information on the admissions process is available here
- Over 90% of students at PLU receive financial assistance
to attend, with the average award being 17,900 dollars.
3 > Complete the FAFSA form
(Free Application for Federal Student Aid)
- The FAFSA can be secured from your high school counselor
or here. The results
of this form are the basis of the awarding of financial
aid. When submitting information, please be sure that
PLU is designated ( code number -- 003785 ) to receive
your processed form.
4 > Have two coaches fill out the
Coaches' Evaluation Form

- Print two copies of the form and have two of your coaches
fill them out.

5 > Send us one video/DVD that
includes highlights of your best plays and/or your best game
from your senior season
- Please send us a duplicate copy, as we are unable to
return tapes. Because it is hard to find players on a
video we have never seen before, we would greatly appreciate
you indicating your name, helmet and jersey color, number,
and exact position on the label of the video tape.
Note: we recommend that you do not send the video
in a "fiber filled" envelope. These envelopes consistently
break open by the time that we receive them and the "fibers"
get all over the tape, making it unviewable. Thank you.
6 > Set up a campus visit
- Contact our admissions department at 1-800-274-6758.
We will work directly with admissions through this process.
