Coordination of Care Objective- Collaborate in the design, management, and coordination of care. Current Coordination of Care In NURS 220 I have been part of the coordination of care with another nursing student . Myself and the other SN worked together to organize morning care in an assisted living facility. In NURS 270 I have been part of a pilot
program that is designed to help CHF patients better manage their disease. Through my feedback I have been able to provide the coordinator with strengths and weaknesses of the program. This has helped the coordinator to make changes to the program in order to better serve the clients as well as help the student nurses do their job. Future Coordination of CareAs a Sophomore II being involved in the design, management, and
coordination of care has been limited but in NURS 499 and NURS 420 I will
gain more exposure. |
