
Evidence of Collaboration
» Case studies in both Nursing 260 and Nursing 270 Assessment were my first experience with collaboration of care. In these groups we used the nursing process to come up with a nursing diagnose and a care plan for that case.
» My second evidence of collaboration is my work with a heart failure client. In this program, I coordinate with the Heart Failure Nurse Navigator, the patient and their doctors, in an indirect way thought the client. Together with the client and with the help from the navigator, if needed, a plan of outcomes and goals is established to comply with the doctor's orders. These include exercise plans, salt intake, liquid intake, weight management and the medication regiment. Please click Heart Failure Client to learn more.
Future classes to increase my experience:
In future classes I hope to learn and provide more evidence of my ability to collaborate. I plan to accomplish this goal with the clinical experience and in the classroom setting. Group work with projects is a great example of ways to increase my evidence for this ability

Purpose: The purpose of collaboration is to provide the best and most complete care for the client, family or group. By combining ideas, skills, and knowledge, a collaboration can be thoughtfully designed, managed well, and coordinated to meet each cases needs. As a nurse it is our responsibility to be patient advocates. By participating in collaborative care we can be the voice for our patients.
