End of Life Care

Friday, June 6, 2008,  8:30 am - 4:30 pm
Are you honoring patient's care wishes for end of life care? Are you relying on outdated policies, procedures and forms? Have you heard of POLST? POLST (Physician Order Life Support Treatment) has been in use for over 6 years to support patient end of life wishes and continuity of care. You need to be prepared to assist those who live for a prolonged period of time with a fatal disease prior to their death. Fatal diseases are not confined to cancer, but include congestive heart failure, chronic obstructive pulmonary disease, end stage renal failure, liver failure, etc. The period that marks the end-of-life is often a long and potentially positive period where appropriate care can enhance relationships, support the family, alleviate pain, and promote spiritual growth. No matter who you care for or in what setting, you will leave this work shop with a new perspective on End-Of-Life Care as well as the skills to make a difference in this very important season of life.
