RN Re-Entry Clinical Experience

Cost: $525.00

Within this range of dates,
participants will complete 160 clock hours as a clinical experience with an
assigned clinical preceptor. Day and evening shifts will be used for
clinical experience on any day of the week, including holidays.
Clinical hours and days will vary for each participant. It is essential
that participants are available to work preceptors' schedules.
Participants who have been inactive in nursing practice for more than
three (3) years can expect to be assigned to extended care facilities
for a portion of the clinical experience.
This program is presented in cooperation with health care facilities in
the South Puget Sound area. The clinical experience is open to persons
holding active Washington RN licenses without encumbrance or
disciplinary action.

APPLICATION PROCEDURE:
1. Request an application packet from CNE. Complete the application packet, including:
- Course Application
- Personal Health History
- Release and Hold Harmless form
- Washington State Patrol Disclosure Affidavit
- Fee for Washington State Patrol Criminal History Record Revue
- Addendum to Application
- Course Application (contact us to request an application)
- Check for $525.00 for course fee made payable to PLU - CNE
2. Mail or bring the application packet and appropriate fees by noon on April 9, 2009 to:

Please be prepared to show your license and original documentation to
the staff of the School of Nursing. Participants will not be cleared
for the clinical experience until the clinical prerequisites are met.
CLINICAL PREREQUISITES:
Applicants must comply with the following requirements by May 1, 2009 to be cleared to enter the clinical experience. The health history form is to be
submitted with your application.
- Hold a current license to practice registered nursing in the State of Washington with no encumbrances.

Show evidence of liability (malpractice) insurance coverage for a minimum of $1,000,000/$3,000,000.

Show evidence of a major medical, accident or sickness insurance coverage.

Completion of a CPR for Health Care Providers course or CPR for the
Professional Rescuer that remains valid throughout the clinical
experience.

Complete Addendum to Application

Complete the Washington State Patrol Disclosure Affidavit

Complete and sign Release and Hold Harmless Agreement

Completion of School of Nursing Health History form and current screening

and immunization requirements to include proof of:

Measles, Mumps, Rubeola (MMR)

Hepatitis B Antibody Titer
-
Chicken Pox (Varicella) Titer
- Successful completion of all
Re-Entry Skills Review Laboratory sessions as evaluated by the
instructors.
- Be available for clinical experience on day and evening shifts any day of the week, including holidays.
REFUND POLICY:
Registration fee, less $20.00 handling charge, will be refunded only
upon written request received by noon on May 1, 2009. A full refund
will be made for cancellations by the University when an enrollment
limit is reached or for insufficient enrollment. PLU reserves the right
to cancel any offering when registration is insufficient to meet
requirements and to set limits for numbers of participants for optimum
teaching/learning conditions.
