No waivers will be accepted after September 19, 2008, midnight.

Care and Coverage

The PLU student health insurance coverage begins your first day of class, or first day of fall orientation (if you attend). Coverage terminates at the end of the policy period, or upon withdrawal or termination from the university, whichever comes first. The primary care provider will be the PLU Health Center. A referral is required for payment of outpatient benefits. When the Health Center is not open or the student is away from campus, during summer for example, the insured student may submit a claim form directly to the insurance company for reimbursement of covered medical expenses obtained through his or her local medical clinic, emergency room, or physician according to benefits schedule.

This plan is an excess plan. That means it pays only after all other possible insurance has paid. It can cover the cost of deductibles and costs not covered by your primary insurance carrier. In the absence of other insurance, this plan acts as primary.

An accident or injury must be treated within 90-days. Claims will be accepted up to 180-days following accident, illness, or injury. Instructions for claiming reimbursement of your medical expenses is online at EIIA Student Program Website.
