Application Process


The following steps will take you through the HPRB approval process.

  1. Review the HPRB Process Overview for students and faculty/staff/admin.
  2. Complete and sign the Proposal Form.  This is a fillable Word form (you may need to enable macros in your Word software to use the form).  All forms must be typed. 
  3. If applicable, include an informed consent document, following this model.  If possible, fit your form to one page.  Be sure to choose EITHER anonymity or confidentiality (not both).  If you are conducting an anonymous survey, you do not need signed informed consent.  Instead, use a cover letter that follows this model.
  4. Attach all additional materials, such as outside approvals, questionnaires, advertisements, debriefing forms.
  5. Make the necessary number of hard copies of your complete proposal (form and all attachments).  Submit two hard copies for expedited reviews or five hard copies for full board reviews.
  6. Students: submit the hard copies to your faculty sponsor.  All other researchers: submit the hard copies to your unit designate (in the Division of Social Sciences, proposals should be submitted to Carol Reed, who will record your submission and then forward it to the appropriate unit designate).
  7. Submit an electronic copy of your complete proposal (form and all attachments) to the HPRB Chair (hprb@plu.edu).   Any attachments that are not available in electronic format should be indicated in the text of the email.
