Payment for the Creating Safer School Laboratories conference can be made via check, purchase order or credit card. Checks should be made payable to Pacific Lutheran University and indicate the participants name somewhere on the check or check stub. Please send the checks attention to: Lab Safety Workshop, PLU, Finance and Operations, Tacoma, WA 98447. A street address is not required. If you will be using a purchase order, we will send an invoice upon purchase order and registration receipt. We accept Visa and Mastercard credit cards. If you will be using a credit card, please feel free to contact us and we will take your credit card information and have the Business Office process the purchase immediately. You may also include your credit card information on the registration form. We will need the following information: Visa or MC, expiration date, card holder name, card holder daytime phone, and amount.Payment Information
