Nursing Process

Program Outcome:

To apply the nursing process in the provision of direct and indirect care of individuals, families, and communities in a variety of settings and across diverse populations.


Clinical Experiences~

During the past three years I have experienced giving care in many different communities and populations.  I was able to apply the nursing process for children, adults, families, as well as entire communities.  The nursing process involves assessment, diagnosis, outcomes/planning, implementation, and evaluation.  These steps can be taken for any patient or population a nurse is caring for, and below are examples of how I used this process in the care I gave.

Applying the nursing process in the care of individuals:

In my Nurs270 class, "Health Assessment and Promotion", I completed a thorough, head to toe assessment on a classmate, pinpointing this person's health issues, and coming up with nursing diagnoses.  Below is the link to this assessment:

Nurs340 (Adult Health I) & Nurs440 (Adult Health II).  In both medical surgical classes, I learned how to safely and professionally apply the nursing process to adult patients who had a wide array of health issues, including chronic pain complications, new medical diagnoses, surgical procedures, and oncology considerations.  Below is a form I used to fill out and organize patient data, in order to work through the nursing process for each of my patients:

Nurs350 (Mental Health).  In mental health, I was able to assess the mental status of my clients, develop a diagnosis, and goals for their care.  Below is a link to one of these assessments:

Nurs499 (Preceptorship - ER).  In my senior preceptorship at the St. Francis emergency department, I developed specific, individualized goals for myself, which are linked below.  These goals helped me to come prepared each day to focus on an area I needed improvement in.  I also have included a link to a journal I wrote while in the ER, which exemplifies the type of patients I've cared for, and the complexity needed to tailor the nursing process to each patient.

Also in my preceptorship, I chose an issue of which was relevant to the emergency department, and constructed a poster that elaborated the nursing process, and how to give the best nursing care for this population.  I chose to do nursing care of the asthma exacerbation patient in the ED.  Below is the link to this poster.

Applying the nursing process in the care of families:

Nurs370 (Childbearing-OB).  In my obstetric class, which included care of the laboring woman, the newborn, and the postpartum couple, I created care plans for this population.

Nurs380 (Childrearing-Peds).  In my pediatric rotation, I gained experience in altering the nursing process and care plans to be focused towards the specific age group of that patient.  Whether a toddler, an infant, or a teenager, the nursing process can take into consideration the growth and development for each, and I was able to formulate plans and goals based on age.  Below is a reflection of my time in pediatrics:

Applying the nursing process in care of communities:

Nurs430 (Community Health).  In my Community Health course, we took the nursing process to a specific community, in this case it was five Adult Family Homes in Gig Harbor, WA.  There, my group and I assessed this population's needs, came up with diagnoses, and created a Fall Protocol, an in-service teaching for the caregiver staff, and a couple of flowcharts for them to refer to in the case of an incident.

As I start my residency in the emergency department, I will continue to use the nursing process to plan and individualize the care of each patient I care for.

Links to geriatric assessments

In my nursing 220 class, I completed a functional and a pain assessment for a resident I was caring for.  The functional assessment assessed the resident's ability to perform activities of daily living, and the pain assessment revealed what sort of acute and/or chronic pain this resident was in.

Cultural Health Care



In my class entitled Cultural Health care, I was able to expand upon my knowledge of diverse populations.  We studied different cultures, and their beliefs about health, relationships, sanitation, and more.  This class helped me to tailor the nursing process to all types of patients who come to me in need of care.  Below are links to a paper I wrote on the Hmong culture, as well as a paper I wrote on the Islamic culture in America, based off an interview I conducted with a young man who had recently moved to the US from Egypt.
