Selected Evidence to Meet Outcome

hands Collaboration is essential to successful nursing outcomes.  My mental health teaching project meets this outcome by showing how i designed a teaching session, coordinated with other health care workers, and managed the class.  I also collaborate in an acute care setting during my Senior synthesis, and my Senior Capstone project illustrates me using current data to design care that incorporates managing a disease as a nurse and coordinating the care with other members of the health care team. 

Reflections


I feel like I came into nursing prepared for situations that required collaboration.  I was in choir, and in those classes throughout high school and college I developed the self confidence to manage other people and facilitate discussions while a group of people are focused on me.  That being said, I still had some anxiety when I became the manager in patient care.

I collaborated, designed, and managed patient care in the acute care setting for the first time in mental health.  I initially discussed my project with the instructor, then collaborated with the occupational therapist as to when we should plan for my relaxation project, what materials I could use, and where I could perform my project.  I had to manage the patients to assure they were participating in the project.  I recall being nervous before the exercise, but once I began I relaxed into my role.  I realized I  could not have completed this venture on my own, and heavily relied on assistance from the occupational therapist.


By my senior year, I felt I learned a lot about collaborating with others in coordinating and designing care for patients.  Our change project changed that perception.  There were numerous people with whom to speak, and we learned that there are many official people who must be made aware of the potential collaboration.  It was a highly educational experience that a true collaboration is better between perceived equals.  It is essential to thoroughly research a situation so as to be considered knowledgeable on a subject.  When there is authority and respect, the collaboration becomes more equal and other members are able to trust in my ability to design and manage care.  Although the project was ultimately unsuccessful, I feel that I have learned extensively what I need to do assure successful collaboration at the management level, and how much collaboration is needed in the design of care, especially if it is to be implemented.  I have to appear that I am bringing a fresh, unique perspective, that I am intelligent and I can equitably contribute in giving care. 

In my senior synthesis I feel I am beginning to embrace the role of a new nurse.  In that role I collaborate with many members of the health care team.  In a typical day on the floor, I collaborate with the doctor to design the patient's care, collaborate to receive orders for the patient's care and to determine when a patient will be discharged.  

When needed I will delegate the Health Unit Coordinator to arrange labs to be drawn and various tests ordered.  In the HUC's absence I will coordinate with the nurses in various settings such as surgery to coordinate care.  

Pharmacy is an excellent source when medications need to be managed, and will sometimes be invaluable in assuring the correct dosage of medications.  

My capstone project is on the care of an asthma patient in a hospital setting.  Asthma is a chronic disease whose care should be nurse led.  The nurse collaborates with the prescriber to determine the most effective medications, coordinates with community resources so the patient may be appropriately referred, and the discharge teaching is mainly designed by the nurse.

Future Plans

I have learned throughout my tenure that health care is a collaboration of many individuals in a hospital, and as a nurse I feel we are the best choice to lead the dialogue in designing and managing patient care.  This is why it is important to be active in policy and procedures committees, and to obtain a position in higher management. 

Initially in my career I plan on collaborating continually with doctors, physician assistants, nurse practitioners and other ancillary staff to assure my patient's care is managed.  Once I have established myself in an area where I plan to specialize, I will seek promotion to influence care.  I also plan on joining committees that oversee patient care and will be a vocal advocate supporting the role of the nurse in health care along with patient rights.  Patients and nursing are in a mutualistic relationship, and to best care for the patient the nurse must be able to effectively exercise her autonomy and clinical judgment to be an equal collaborator in the care of the patient.

Mental Health Teaching Project This project was designed to discuss stressful triggers in the patients, and then give helpful tips to aid in relaxation and perform a meditation exercise.

Senior Capstone Project This project is a compilation of the standard care to give a patient with asthma, and many of the interventions are collaborations with other staff.  Included is managing the care, being the main point of information, and assuring effective care.
