Selected Evidence to Meet Outcome The evidence meets my outcome goals by showing application of the nursing process for individuals, families and communities. There is both direct and indirect care involved. The mother baby unit is in an acute care setting, where the patient is fairly isolated from other patients on the floor. The mental health patient is also in an acute care setting, but has a longer stay in the hospital, the care is highly structured and integrated with other patients. The community health project provides indirect care in the community setting. My record of clinical practice demonstrates the different patients I have cared for, directly and indirectly, as well as the diversity of my patient's ethnic and cultural background. ReflectionsThe nursing process is very similar to the scientific process. The scientific process I had experience with in my prerequisite courses, such as chemistry, biology, and psychology. In the process, we research and assess a particular situation. I come up with a hypothesis, or nursing diagnosis. I label what goals and outcomes I want to accomplish, and plan an, implement. Finally I evaluate the outcome to determine if the original outcome was met. While I understood the process, I had a difficult time translating exactly what needed to be done.In addition, there was a lot of paperwork that needed to be prepared to illustrate the nursing process in order for me to understand it. This type of thinking does not come naturally, and I think the more explicit care plans I wrote in the beginning do well to illustrate the process behind the care I give in clinical. In working with different populations, I have realized that I am still able to utilize the nursing process in my practice. In my senior synthesis, even though we do not write care plans for our clients, in our documentation and assessments we are performing the nursing process daily. Upon admission, I do a full history and physical, and from there I fill out a care plan. During the patient's stay, we have interventions to perform and at discharge we review the plan to determine if the outcomes are met. This setup isn't exactly like our nursing care plans but it is a similar thought process. I am by no means proficient at applying the nursing process. I am beginning to look at patient care holistically, but I get distracted by the tasks needed to be accomplished and occasionally don't think as critically as I should. Oftentimes it is in retrospect I will catch something, but I am responding positively to feedback. I have also worked with many patients with diverse populations. I have extensive history in performance arts, and my previous work experience allowed me the opportunity to work with numerous ethnic and cultural backgrounds, so I feel comfortable providing care that is based on customer service principles and that is culturally congruent. Future PlansI plan on continuing my habit of reflecting on patient care after I have graduated and beginning my clinical practice. I feel it is invaluable to spend time pondering what I did during the day, what went well, what did I miss, and what can I do better the next day. I plan on continually seeking out different patient populations, and reflect on what my perceptions of race are, and how I can effectively give care based on my preconceived notions. I strongly believe that if you treat everyone with courtesy and respect that you should do well, but I need further immersion in the health care field to determine if that practice will continue to be effective. I hope to continue to work with communities to utilize the nursing process to provide both direct and indirect care, as I truly believe nursing is most beneficial when supporting public health initiatives, and focus on at risk and minority populations. I had a lot of fun volunteering for homeless connect, and plan to continue to volunteer for that event after graduation and yearly. |
