The Non-PLU form may be submitted after May 1, but non-PLU students will not be
registered by the Summer Sessions Office until May 15, 2009.
Students
need to complete this form to gather all the information that will be
required for a summer session registration. Fax this form to

Registration is based on first-come, space available basis.
All forms are stamped in the Summer Sessions Office as
of date received.
