For community health it is a requirement that all PLU students, faculty, and staff be fully vaccinated against COVID-19. Boosters are strongly encouraged.

Students

PLU requires all students—except students in online-only programs—to be fully vaccinated against COVID-19 before arriving on campus, with case-by-case exceptions made for medical or religious/right of conscience reasons.

For participation in classes and activities, students must submit their documentation or file for an exemption with the Health Center.

3 Steps to Upload Vaccination Verification

  1. Go to our Etrieve Portal website: Student Form
  2. Enter your PLU ePass login username and password
  3. Follow the steps to indicate vaccination booster status and upload your documents (please note that verification takes approximately ten business days).

If you are a student employee you only have to submit your material through the student form.

If you have already submitted your initial COVID-19 vaccination material, you are only required to submit your booster information.

If you feel that you are ineligible for the booster because of severe side effects from the initial series, please contact your healthcare provider to request a medical exemption using the PLU Medical Exemption form.

If you have received your vaccination in Washington state you can obtain a digital COVID-19 vaccination record through the Washington State Health Department verification website.

Faculty and Staff

We require COVID-19 vaccination for faculty and staff as well as students. This requirement is key to remaining a healthy and safe campus and consistent with public-health guidelines. Exemptions may be requested for medical reasons and religious beliefs, including right of conscience.

  3. Follow the steps to indicate vaccination booster status and upload your documents (please note that verification takes approximately ten business days).

If you are a student employee you only have to submit your material through the student form.

Even if you have already submitted to Human Resources, we would like you to resubmit the information.

If you feel that you are ineligible for the booster because of severe side effects from the initial series, please contact your healthcare provider to request a medical exemption using the the Vaccine Exemption Request form (staff/faulty).

If you have received your vaccination in Washington state you can obtain a digital COVID-19 vaccination record through the Washington State Health Department verification website.
