PLU is hosting a flu shot clinic at no cost for ALL faculty, staff, and administrators. No appointment is necessary.

November 1, 2012 – Thursday10:00 a.m. – 2:00 p.m.Anderson University Center, CK West

No cost immunizations are being provided to all employees; however, please bring your PLU Group Health insurance card and photo identification if enrolled.

You will be required to fill out a Patient Consent Form. You may fill out the form ahead of time or on the day of the clinic. Download the attached form or visit the Human Resources home page https://www.plu.edu/human-resources/. If you have questions, please call Human Resources at ext. 7185.
